The fixed, up-front dollar amount you pay for certain covered expenses. Office visit co-pay amounts do not apply toward your deductible or co-insurance, and they do not accumulate toward the out-of-pocket limit.

The percentage of claim cost you pay after the deductible has been met. The co-insurance accumulates to the maximum out-of-pocket amount.
Back to TopWhat is maximum out-of-pocket?
The maximum out-of-pocket limit includes the deductible, co-pays and co-insurance. After an individual has satisfied this amount, the health plan covers all eligible, in-network health care expenses, including co-pays, for the rest of the plan year.
Back to TopWhat is the difference between deductible and family deductible?
An individual deductible is the amount an individual must meet before benefits begin for services not covered by co-pays. Once this amount is met, the plan will begin paying claims for that individual at the co-insurance level.
The family deductible is the amount family members must meet in order for the plan to consider all family members to have met their deductibles. One individual cannot contribute to the family deductible more than the individual deductible.

Travel immunizations are covered at 100 percent with no deductible. Injectable, anti-malarial drugs are covered under the travel immunizations benefit. If the malaria medication is provided in a pill format, it is covered under the prescription drug benefit.

Health plan participants should receive one ID card from Cigna Global Health Benefits to use for medical and prescription benefits. You should receive your ID card within a few weeks of your enrollment date. If you need to see a doctor or fill a prescription before your new ID card has arrived, please contact your HR department.

The amount you pay for an office visit to a network primary care physician such as a pediatrician, general practitioner, family practitioner, internist or gynecologist.

Any physician not considered a primary care physician is considered a specialist. Chiropractic care is paid according to standard plan provisions (number of visits annually are limited).
